How do I create a form in Google Docs?
1) Login to the Google doc I will share with you OR create a Google docs account.
2) Click where it says “Google Docs Beta” to access your account.
3) Click on “new”
4) Click on “forms”
5) Title your form by clicking in the “untitled form” box and typing your title.
6) Type in survey instructions in the instructions box.
7) To Add a Question
a. Click on the “Add Question” button.
b. Select the type of question you want.
c. Type in your question in the “Question Title” box.
d. If you selected a question type that requires answers, type in your answers.
e. If you want the questions to be required, click on the “Make this a required question” box.
f. Click Done.
8) To Edit a question
a. Click on the pencil next to the question you want to edit.
b. Make the appropriate changes.
c. Click Done.
9) To Copy a question
a. Click on the duplicate button (The box with two squares) next to the question you wish to duplicate.
b. Click on the edit (pencil) button and edit the question.
10) To Delete a question
a. Click on the trash can icon next to the question.
11) When you are finished, click Save (found in the top right hand corner)

How do I see the results of my survey?
1) Log in to Google Docs.
2) Click on the name of the survey.
3) A spreadsheet with the results will appear.

How do I invite people to take the survey via email?
1) Click on “email this form” when you are finished.
2) Enter the email addresses you wish to send the form to.
3) If you are sending the form to more than one person, separate the addresses with commas, please.

How do I add this survey to the desktop of my classroom computer so students may access it?
1) Log in to your Google docs account on the classroom computer.
2) Go to the live form by
a. Clicking on the form’s spreadsheet
b. Click form, go to live form
c. Go to File, send, shortcut to desktop.
(You can do this with any page you want your students to access.)

How can I put a link to the survey on my web page?
1) Log in to your Google docs account on the classroom computer.
2) Go to the live form by
a. Clicking on the form’s spreadsheet
b. Click form, go to live form
c. Copy the form address in the search bar
d. Open your webpage program, and hyperlink an object to that address.
How do I share results with others?
1) Access your account
2) Click on the spreadsheet you wish to share
3) Click Share, Invite People
4) Enter the email addresses you wish to send the form to.
5) If you are sending the form to more than one person, separate the addresses with commas, please.
If you do not want others editing the results, make sure you select “to view”. Also, make sure to select “Always Require Sign In” for confidentiality’s sake.

Can I copy and Edit Surveys? Yes!
1) Log in to Google Docs
2) Click on the sheet/form you want to duplicate
3) Select file, create a copy
4) Rename the sheet, click ok
5) Open your new spreadsheet
6) Select Form, Edit Form
7) Save your changes.
